Please NOTE that ONLY Candidates who are of Lagos State Origin are eligible for screening. Candidates in this category are expected to be CLEARED by the Independent Indigeneship Verification Committee (IIVC) BEFORE being considered for admission.

2. LASU receipt of IIVC payment

3. Online Data Form

4. UTME Form showing choice of Institution with UTME Score

5. O’Level Result

6. Two passport photographs

7. Applicants Birth Certificate

8. Father/Mother’s Birth Certificate

9.Letter from Oba (not Baale) of applicant’s town of origin

10. Letter from Chairman of applicant’s Local Government of origin
